The Reality of AI Governance as a Founder


When you're a founder deploying healthcare AI, you don't have the luxury of 15 governance committees and 200-page policy documents. You need minimum viable governance—the essential controls that protect patients without killing innovation.

But "minimum viable" doesn't mean careless. In healthcare AI, governance failures don't just create compliance problems—they risk patient safety. The challenge is identifying which governance practices are truly essential versus which are enterprise bureaucracy.

Healthcare AI Governance Is Different


Healthcare AI governance has challenges that don't exist in other industries. Here's what I've learned actually operating these systems:

Most AI models are trained on North American and European populations. When you deploy them in African healthcare contexts, they systematically underperform. Clinical validation isn't about testing if the model works—it's about testing if it works for your specific patient population.

This means:

  • Testing on diverse skin tones and phenotypes
  • Accounting for population-specific disease presentations
  • Recognizing when your training data doesn't represent your patients

Challenge #3: Maintaining Oversight Consistency

In theory, every AI output should have human review. In practice, this is operationally challenging—especially when scaling. The governance question becomes: how do you maintain consistent clinical oversight without creating bottlenecks that make the AI unusable?

My approach: risk-based oversight. High-risk outputs (treatment recommendations, diagnoses) always get human review. Lower-risk outputs (educational content, symptom screening) have lighter-touch review with post-deployment monitoring.

Challenge #2: Bias That Isn't Obvious

AI bias in healthcare isn't always dramatic. Sometimes it's subtle—the model is slightly less accurate for certain demographics, or it requires more data to achieve the same confidence level. These subtle biases compound over time, creating systematic disparities in care quality.

The hardest governance challenge: balancing comprehensive controls with startup realities. You don't have unlimited budget, dedicated governance staff, or months to implement perfect processes. You need governance that actually works with the resources you have.

The Non-Negotiable Principle: In all cases, clinical safety trumps speed. If an AI output conflicts with standard care, the human clinician's judgment always prevails. This isn't negotiable, regardless of resource constraints.

Building the Framework: What I Actually Use

  3. Ghana/Nigeria/Kenya data protection requirements — local regulatory compliance

Patient safety in AI systems rests on four non-negotiable pillars:

Technical Governance in Practice


Here's how I actually manage AI models in production:

Every AI model update goes through a validation cycle:

  1. Technical testing — Does the model perform as expected on test data?
  2. Clinical review — Do medical advisors approve the changes?
  3. Controlled rollout — Deploy to small user subset first, monitor closely
  4. Full deployment — Only after validation passes all stages

Monitoring for Model Drift

AI models degrade over time. Input data changes, patient populations shift, or external factors affect performance. I monitor for drift by tracking performance changes against known benchmarks.

If anomalies appear—accuracy drops, error rates increase, or outputs become inconsistent—I pause or roll back the model immediately. This reflects my CISA audit discipline: trust, but always verify.

Cross-Border Governance: One Framework, Local Adjustments

Operating across Ghana, Nigeria, Kenya, and South Africa means navigating:

I design one governance framework, then adjust for each country's specific requirements. Where regulations conflict, I use the strictest standard as the baseline. This ensures compliance everywhere without duplicating governance effort.

Practical Example: Nigeria requires explicit consent for AI processing. Ghana's requirements are more flexible. Rather than maintain two consent systems, I implement Nigeria's stricter standard everywhere. One system, guaranteed compliant in all jurisdictions.


Real Governance Lessons: What I Got Wrong and Right

The Mistake: Underestimating Model Drift

Early on, I underestimated how quickly AI performance can degrade in live environments, especially with the variability of health data. I assumed that if a model validated well initially, it would stay accurate. Wrong.

The Incident: Upstream Data Quality Changes

One incident involved unexpected output variability due to changes in upstream data quality. A data source we relied on changed their collection methodology without notifying downstream users. Our AI model, trained on the old data format, started producing inconsistent outputs.

Governance With Limited Resources: What Actually Matters


As a founder, you don't have an unlimited budget for governance. Here's what provides the highest ROI:

  • Decision logs—simple documentation of who decided what and why

Big-enterprise governance is unnecessary and counterproductive for startups. What matters is

consistency, clarity, and accountability—not bureaucratic overhead.

The challenge isn't building perfect governance. The challenge is building practical governance that actually works with the resources you have, in the regulatory environment you operate, for the patients you serve.

Governance that looks good on paper but fails in practice is worse than no governance at all—it creates false confidence. Governance that works is simple, consistent, and focused on outcomes: patient safety, system reliability, regulatory compliance, and operational accountability.
